Catholic Charities Tioga Outreach Center begins Health Coaching Program

Catholic Charities Tioga Outreach Center is excited to announce the start of its Health Coaching Program funded through a grant by Care Compass Network.  The goal is to assist clients with developing a proactive sense of control over their personal health.  

Individuals who enroll will receive two visits to their home or a site of their choosing by the Health Coach. Several follow-up calls will also be made to see how the client is doing and if they are encountering any problems.  

The role of the coach is to provide education, encouragement and accountability. The targeted health issue will be the choice of the client, not the coach.  

The program is being offered to individuals on certain Medicaid health insurances, namely Fidelis, United Health Care Community Plan and CDPHP.  Individuals without insurance may also qualify. The ultimate determination will be decided based on the results of a quick and easy survey.

The center is open Monday through Friday, from 9 a.m. to 4:30 p.m. You can also meet Presnell at the following times at locations: Tioga County Rural Ministry, 143 North Ave. in Owego, first Tuesday of the month from 10 a.m. to noon; Bread of Life Pantry, old auction barn across from the Post Office, 1 Water St. in Candor on the first Wednesday of the month from 4:30 to 6 p.m.; the Richford Congregational Church on Route 38, the first Thursday of the month from 11 a.m. to 1 p.m.; Red Door Café at 359 Broad St. in Waverly, on the first Friday of the month from 1 to 3 p.m.; and at the Community Food Cupboard, located at the Spencer Federated Church, 70 N. Main St. on the last Thursday of the month from 10 to 11 a.m.
