On March 15, 2016, at about 3:05 p.m., New York State Police at Owego responded to a Tioga County 911 report of a one vehicle fatal crash on Dean Creek Road in the Town of Barton, Tioga County, N.Y.
An investigation determined that the driver, Deborah Lynn Mahoney Reynolds, 41 of Athens, Pa. had lost control of her 2013 Toyota Land Cruiser and had exited the east side of the roadway and struck a tree.
Deborah Lynn Mahoney Reynolds died at the scene from head trauma. Her vehicle was also occupied by her three children, ages four, eight and ten, who were not injured in the crash.
The Tioga County Coroner responded to the scene and directed the deceased to be taken to Lourdes Hospital for an autopsy.
