Are you looking for free guidance on long-term care options or information about services available throughout our community? “NY Connects” is a free information and assistance service that will work with you and your family to create a resource listing that will utilize family, friends, community resources, and local agency supports to help meet your needs.
This program is available to those of all ages. For those with limited English speaking ability, the program uses LanguageLine Personal Interpreter services in order to better understand your needs.
